Dealers must average selling 10 cars per month and have been in business for at least two years.

• Employee lists, title, driver's license #, date of birth
• Loss runs 3-5 years
• Acord 125
• If requesting agreed value, please fill out and have insured sign the forms below
  • Statement of Values Form Building Coverage
  • Statement of Values Form Business Personal Property
  • Statement of Values Form Business Income
• Acord 130 if submission includes WC
• Current NCCI Experience Modifier Worksheet if quote includes Work Comp
